State Forms Payroll Compliance Form
Certain vendors hired by the Town must submit certified payrolls to document their employees were paid the prevailing wage while working for the Town. This form certifies that the payroll submitted on the Weekly Payroll Report Form is accurate and the employees were paid according to the requirements of the Prevailing Wage Program.
